The following online application takes approximately 6-minutes to complete. As part of the process, we'd like to ask for your demographic information. Your information will be kept confidential. None of your responses will be shared outside our organization.

In addition to this:

• You will be required to submit a 10-minute introduction video for Bonnie's review. Details as to what include in your presentation along with a private upload link will be emailed to you.
• Email your bio along with your essay to:  bonnie.bonniewirth@gmail.com, subject line "My Elevate Application"
• Information regarding the 4-month affiliation will be emailed to you. Should you choose to participate and once the agreement has been signed, a pre-program consult will be scheduled with Bonnie to discuss your vision for Elevating others!
